Milestone and Our House Senior Living
Annual Scholarship Fund

The Milestone and Our House Senior Living Scholarship Fund was established in 2006 to coincide with our Team Member Mission - "Our House fosters an environment of respect and integrity while providing opportunities to grow personally and professionally."

As valuable members of the Milestone and Our House Senior Living family, Milestone Management Services designed the Scholarship to recognize one Team Member from each Milestone and Our House Senior Living location who is choosing to pursue a higher education in the health care profession through an accredited school.

We are excited to make this Scholarship available to one team member per Our House and Milestone Senior Living location as we believe the growth of our team members comes from continuing education and life long learning.

Eligibility

• A current, active full or part-time (greater than 20 hours) Our House and Milestone Senior Living Team Member who has successfully completed the orientation period.
• Accepted or registered at an educational institution for full or part-time attendance as defined by the institution.

Application Requirements

• Acceptance letter from the accredited school.
• Completion of one of three essay questions outlined in the application.
• Two letters of recommendations - one must be from a supervisor/ mentor, and one character reference from a non-family member.

Award

Scholarship Amount ----- $1,000.00 net, to be paid in the name of the team member as well as the college.

Application Deadline

Team Members interested in obtaining an application should download the application or request the Scholarship packet from the Human Resources Department at Milestone Management Services, 1574 West Broadway Suite 200 Madison, WI 53713.

All application materials must be received and postmarked by May 1 of this year and mailed to:

All application materials must be mailed as one package.

Awards are for the current academic year and by law are paid directly to the team member. The Milestone and Our House Senior Living scholarship committee will review applications no later than June 15. Team member finalists may be invited to participate in a face-to-face or telephone interview.

Early applications are encouraged. Team members may re-apply each year. Awardees will be notified in writing of acceptance no later than June 30. By June 30, a letter will be sent to all participants informing them of the award winner.
